For many women, it’s hard to tell if these changes are due to Polycystic Ovary Syndrome (PCOS), Premenstrual Syndrome (PMS), or simply the effects of stress. What is PCOS (Polycystic Ovary Syndrome)?

PCOS is a hormonal disorder that affects women of reproductive age. It’s characterized by an imbalance of reproductive hormones, which can lead to a range of symptoms, some of which overlap with PMS and stress.

Key indicators of PCOS include:

  • Irregular or missed periods: This is one of the most common signs, often significantly longer cycles, infrequent periods, or no periods at all.
  • Excess androgen (male hormone) symptoms: This can manifest as hirsutism (excessive hair growth on the face, chest, back), severe acne, and thinning hair.
  • Polycystic ovaries: While not always present, an ultrasound might show enlarged ovaries with multiple small follicles.
  • Weight gain/difficulty losing weight: Often around the abdomen.
  • Insulin resistance: Leading to fatigue, increased hunger, and dark patches of skin.
  • Long-term health issues: Such as type 2 diabetes, heart disease, and infertility.

What is PMS (Premenstrual Syndrome)?

Premenstrual Syndrome (PMS) refers to a group of physical and emotional symptoms that occur in the days or weeks leading up to your period. These symptoms typically resolve once menstruation begins and follow a cyclical pattern each month. PMS is extremely common, affecting up to 75% of women at some point in their lives. While most cases are mild, severe symptoms may indicate a more intense form called Premenstrual Dysphoric Disorder (PMDD), which requires medical attention.

Common PMS symptoms include:

  • Mood swings: Irritability, anxiety, sadness, increased emotional sensitivity.
  • Physical discomfort: Bloating, breast tenderness, headaches, fatigue, muscle aches.
  • Cravings: Increased appetite or specific food cravings.

Could it be Stress?

Stress is a normal part of life, but chronic stress can mimic or worsen symptoms of both PCOS and PMS. When you’re stressed, your body releases cortisol, which can interfere with hormone regulation. The symptoms of stress oftentimes overlap with both PCOS and PMS.

Some common symptoms are:

  • Irregular periods: Stress can delay or even temporarily stop your period.
  • Weight fluctuations: Stress can lead to both weight gain and loss.
  • Fatigue and sleep disturbances: Feeling constantly tired, difficulty falling asleep.
  • Digestive issues: Upset stomach, changes in bowel habits.
  • Acne: Stress can trigger breakouts.

How to tell the difference?

  • Duration: Stress-related symptoms can come and go and may be linked to life events or ongoing pressures.
  • Frequency: If symptoms persist regardless of your cycle, consider PCOS or chronic stress as possible causes.
  • Impact: Both PCOS and chronic stress can affect fertility and long-term health, while PMS typically does not unless it is severe (PMDD).
